An Intensive Upper Extremity Program Coupled With a Wearable Device for Poststroke Rehabilitation: A Qualitative Study of the Perspectives of People With Stroke.

When rehabilitation programs move online and add wearable sensors, it matters whether people actually find them workable in daily life, not just whether the technology performs well on paper. This qualitative study looked at the experiences of people with stroke who completed the Virtual Arm Boot Camp, a three-week virtually delivered arm exercise program combined with a wearable device that gave feedback on how much the affected hand was being used, alongside therapist support. Researchers interviewed nineteen participants after they finished the program and analyzed what they said about it. Three themes emerged: the program felt motivating, the technology setup was accessible but sometimes challenging to manage, and using the affected hand more in everyday life was possible but with mixed success across individuals.
Because this is a qualitative study of one group's experience rather than a trial comparing outcomes, it cannot show whether the program improved arm function, only how participants experienced it.

This qualitative descriptive study was embedded within a multicenter randomized controlled trial on the CanStroke Recovery Trials Platform, examining participant experience with the Virtual Arm Boot Camp, a three-week telerehabilitation program pairing intensive upper extremity exercise with a wearable device delivering reach-to-grasp feedback and remote therapist support. Semi-structured video interviews were conducted with nineteen participants (mean age 60.2 years, SD 12.2; mean 217 days post-stroke, SD 109.2) after program completion, and data were analyzed using conventional content analysis, with trustworthiness supported by triangulation, analytic meetings, peer examination, and reflexivity. Three themes were identified: the program provided motivating practice, the remote equipment and technology setup enhanced accessibility but introduced setup and usability challenges, and increased paretic hand use translated into daily life with variable success across participants.
As a qualitative study of experience within a single program, findings speak to acceptability and implementation barriers rather than functional or clinical effectiveness, and cannot be generalized as evidence of treatment benefit. These insights are intended to inform refinement of telerehabilitation and wearable-integrated program design rather than to establish efficacy.
