The effect of a 12-week tele-exercise using immersive virtual reality on functional capacity in adolescents with cystic fibrosis: A randomized controlled, single (assessor) - blind study.

Adolescents with cystic fibrosis often have lower physical activity than their peers, partly because infection risk limits group sports and in-person exercise programmes. This trial looked at whether adding immersive virtual reality to remotely supervised tele-exercise could help, comparing a virtual reality tele-exercise group with a standard tele-exercise group over 12 weeks of group sessions held three times a week. Both groups had similar characteristics at the start. After training, the virtual reality group walked significantly further on a six-minute walk test than before training began, but lung function, muscle strength, and quality of life did not show statistical improvement in either group.
Feasibility, enjoyment, adherence, and side effects were similar between groups. This was a small study of 21 participants who completed it, and the improvement in walking distance was a within-group change rather than a direct comparison showing virtual reality outperformed standard tele-exercise, so the findings should be read as encouraging but preliminary.
If you're a teenager with cystic fibrosis, you may already know that avoiding crowded gyms or sports halls because of infection risk can make it harder to stay active. This study tested whether doing exercise at home through a video call, with some participants using an immersive virtual reality headset, could help teens with cystic fibrosis build up their fitness over 12 weeks. Both groups exercised in groups of six, three times a week.
The group using virtual reality showed a meaningful improvement in how far they could walk in six minutes by the end of the programme, but muscle strength, lung function, and quality of life scores didn't show clear improvement in either group. Both approaches were similarly easy to stick with and enjoyable, and no notable safety concerns came up. This was a small study with only 21 participants finishing it, and it can't tell us for certain that virtual reality worked better than plain tele-exercise, only that the virtual reality group improved over the course of the programme.
Longer programmes might be needed to see changes in strength or quality of life.

This trial addresses whether remotely delivered exercise, specifically immersive virtual reality tele-exercise, offers any advantage over standard tele-exercise for improving functional capacity in adolescents with cystic fibrosis, a population where infection control concerns limit conventional group-based rehabilitation. Twenty-one participants aged 12 to 18 completed the 12-week programme, randomised to virtual reality tele-exercise or tele-exercise alone, with sessions delivered in groups of six for 30 minutes, three days weekly. The primary outcome, six-minute walk distance, improved significantly within the virtual reality group post-training compared to pre-training (p=0.003), but the abstract does not report a statistically significant between-group difference favouring virtual reality over standard tele-exercise.
Secondary outcomes, including FEV1, hand grip, shoulder flexor and knee extensor strength, and Cystic Fibrosis Questionnaire Revised scores, showed no statistical improvement in either group. Feasibility, enjoyment, adherence, and adverse effects were comparable between arms. For practice, this suggests tele-exercise, with or without virtual reality, is feasible and acceptable for this population, and the within-group walking distance gain in the virtual reality group is a useful signal rather than confirmed superiority.
The small completer sample, single-assessor blinding, and lack of clear between-group separation on most outcomes limit how far these findings can currently inform decisions about which delivery mode to prioritise.
