Efficacy of physiotherapy treatment in medium and long term in adults with fibromyalgia: an umbrella of systematic reviews.

Researchers looked at 33 systematic reviews of physiotherapy treatments for fibromyalgia in adults to see what actually works over medium and long periods. They examined both active approaches, like exercise and movement therapies, and passive treatments, like manual therapy and devices that stimulate nerves. The overall finding was straightforward: physiotherapy showed positive effects on the main problems people face, including pain, reduced physical capacity, and poor quality of life.
However, the picture is incomplete. Most studies showed benefits, but the researchers found a real gap in the evidence: nobody is describing their treatment programs clearly enough, and almost nobody is following people long enough to know if improvements stick around.
This review looked at what research shows about whether physiotherapy actually helps fibromyalgia over the longer term. The good news is that studies across many different approaches, from exercise and yoga to manual therapy and other hands-on treatments, found improvements in pain and physical function.
People in these studies tended to feel better and do more. The less clear part is that scientists still don't have a detailed playbook.
They don't have enough information about exactly how often people should come, how hard the work should be, or for how long treatment lasts before changes wear off. Long-term follow-up is mostly missing, so it's hard to know whether gains remain stable months or years later.

This umbrella review synthesises evidence across 33 systematic reviews and finds a consistent positive signal for physiotherapy across both active and passive modalities in fibromyalgia management. The breadth is reassuring: stretching, tai chi, yoga, Pilates, hydrotherapy, aerobic training, multidisciplinary approaches, manual therapy, rTMS, and other modalities showed beneficial effects on pain, physical capacity, and quality of life markers.
The methodological quality of included reviews was moderate (mean PRISMA score 18.6 out of 32), and the authors appropriately hedge their conclusions. The real limitation is operational: treatment protocols lack specificity around frequency, intensity, and dose, and follow-up periods remain too short to assess durability of benefit.
This creates a gap between knowing something works and being able to standardise how it works, limiting the translation from evidence to reproducible clinical practice.
