Effectiveness of shoulder injury prevention programs in an overhead athletic population: A systematic review.

This review pulled together research on shoulder injury prevention programs for overhead athletes, meaning people like baseball pitchers, volleyball players, or swimmers who repeatedly load the shoulder in throwing or overhead motions. The researchers searched several major databases through the end of 2020 and found only seven studies that fit their criteria, which already tells you the evidence base here is pretty thin. Of those seven, only three actually showed a favorable effect on reducing shoulder injuries.
Two studies looked at strengthening and flexibility exercises together and found a reduction in injury odds, and one study found that stretching the back part of the shoulder capsule was linked to fewer injuries over time. But here's the catch, only one of those three studies was judged to be at low risk of bias, meaning the others had some methodological weaknesses that make their results less trustworthy. Given all this, the authors were careful to say that no firm conclusions can yet be drawn about whether these prevention programs actually work.
It's a case of promising early signals rather than settled science.
If you're an overhead athlete, someone who throws, serves, or spikes as part of your sport, you may have heard about shoulder injury prevention exercises meant to keep your shoulder healthy. This review looked at the actual research behind those programs and found that the evidence is still quite limited.
Only seven studies met the bar for inclusion, and just three of them showed any reduction in shoulder injuries. The programs that showed some benefit involved shoulder strengthening and flexibility exercises, and separately, stretching aimed at the back of the shoulder joint. These are interesting findings, but only one of the studies showing benefit was considered high quality, so it's hard to say with confidence how reliable these results are across different athletes and sports. What this means is that the science on shoulder injury prevention for overhead athletes is still catching up to the popularity of these programs.
It doesn't mean the exercises don't help, it means researchers haven't yet gathered enough solid proof one way or the other. This is a case where waiting for more research feels honest rather than disappointing.

This systematic review searched CINAHL, Embase, PubMed, and SPORTDiscus through December 2020 and identified only seven randomized controlled or prospective cohort studies examining shoulder injury prevention programs in overhead athletes with injury incidence as an outcome. That's a small pool to draw conclusions from, and it reflects how underdeveloped this area of research still is relative to how often these programs are used in practice. Of the seven, two studies reported odds ratios showing reduced shoulder injury following strengthening and flexibility programs (OR 0.72, 95% CI 0.52 to 0.98, and OR 0.22, 95% CI 0.06 to 0.75), and one study reported a hazard ratio of 0.36 (95% CI 0.13 to 0.95) for reduced injury following posterior capsule stretching.
These numbers suggest a meaningful reduction in some studies, but the confidence intervals are fairly wide, particularly for the second odds ratio, which signals real uncertainty around the true effect size. Only one of the three favorable studies was rated at low risk of bias, and the authors explicitly state that no conclusions can currently be made about the effectiveness of these prevention programs. This points to a clear need for higher quality, adequately powered trials before the evidence can meaningfully guide practice decisions around shoulder injury prevention protocols in this population.
