Psychosocial interventions help facilitate recovery following musculoskeletal sports injuries: a systematic review.

Recovering from a sports injury isn't just about tissue healing, mindset matters too, and researchers have increasingly wondered whether directly addressing psychological factors during rehabilitation could help athletes recover more fully. This systematic review looked at studies testing psychosocial interventions, things like relaxation and guided imagery, positive self-talk, goal setting, counseling, emotional disclosure, and modeling videos, in athletes recovering from musculoskeletal injuries. Across eight included studies, these interventions were associated with improvements in mood, pain management, exercise compliance, and adherence to rehabilitation programs. That's a promising signal, but the evidence base is small and notably, none of the included studies actually examined whether these interventions affected return to play itself.
So while psychosocial strategies appear linked to better recovery experiences and rehab engagement, whether they translate into athletes actually getting back on the field remains an open question the current research simply hasn't answered.

This review looked at whether structured psychological strategies, like relaxation techniques, positive self-talk, goal setting, counseling, or writing about your feelings, could help athletes during recovery. Across a small number of studies, these approaches were linked to improvements in mood, better pain coping, and more consistent follow-through with rehab exercises.
That's encouraging in a narrow sense, but here's an important gap: none of the studies actually tracked whether these strategies helped athletes return to their sport. So while there's some indication that addressing the psychological side of recovery supports the rehab process itself, whether it actually speeds up or improves return to play is simply unknown from this evidence. The review is also based on a small collection of studies, so these patterns should be seen as an early signal rather than a settled finding.

This systematic review, following PRISMA methodology, identified eight studies evaluating psychosocial interventions in athletes recovering from musculoskeletal injuries, drawn from searches of PubMed, Embase, and Google Scholar. Interventions included relaxation and guided imagery, positive self-talk, goal setting, counseling, emotional or written disclosure, and modeling videos. Across these studies, interventions were associated with positive mood changes, improved pain management, better exercise compliance, and greater rehabilitation adherence.
However, the review does not report pooled effect sizes, confidence intervals, or a formal risk-of-bias assessment across studies, and with only eight included studies the evidence base remains limited in scale and heterogeneous in intervention type and outcome measurement. Critically, no included study assessed return to play as an outcome, so despite improvements in proximal psychological and behavioral measures, there is no evidence linking these interventions to the ultimate outcome clinicians and athletes often care about most. The authors call for further research to clarify optimal intervention types, timing, duration, and delivery methods, and specifically to determine whether addressing psychosocial factors during rehabilitation influences return-to-play outcomes, a question this body of evidence does not yet answer.
