Effects of Lifestyle Modification Interventions to Prevent and Manage Child and Adolescent Obesity: A Systematic Review and Meta-Analysis.

Childhood obesity prevention and treatment programs come in many forms, mixing diet changes, physical activity, and behavioral counseling, and this review pooled evidence from hundreds of studies to see which combinations actually move the needle on weight-related measures in children and adolescents. Researchers compared various lifestyle interventions against control groups across both obesity prevention and obesity management contexts, drawing on 654 studies and well over a thousand papers, mostly from high income countries. For prevention, combining diet and exercise showed small reductions in BMI z-score, BMI, and body weight compared to controls, though the quality of this evidence was rated low and results varied a great deal between studies. For management, exercise alone and combinations involving diet, exercise, and behavioral therapy showed somewhat more consistent reductions in BMI z-score, with moderate quality evidence in several cases.
The central limitation running through this review is the low certainty of much of the evidence and the near-total absence of data from low and middle income countries, despite obesity being a growing concern there too.

This systematic review and meta-analysis pooled 654 studies, split roughly evenly between obesity prevention (359 studies) and management (280 studies), with some overlap. For prevention, diet plus exercise reduced BMI z-score by a mean difference of -0.12 (95% CI -0.18 to -0.06) across 32 studies and over 33,000 participants, but heterogeneity was very high (I2 93%) and evidence quality was rated low.
Similar patterns held for BMI and body weight outcomes in prevention, with low quality evidence throughout and I2 values reaching as high as 100%, indicating substantial inconsistency across included trials that tempers confidence in pooled estimates. For management, exercise-only interventions showed a more consistent effect on BMI z-score (MD -0.13, 95% CI -0.20 to -0.06, I2 0%, moderate quality), and several combined interventions, including diet with behavioral therapy and diet plus exercise plus behavioral therapy, also showed moderate quality evidence with lower heterogeneity than the prevention studies. Risk of bias was not detailed in the abstract beyond quality ratings.
The most striking limitation is geographic: 81% of studies came from high income countries and only 2% from low-middle-income countries, leaving a substantial evidence gap for regions facing rising obesity burdens.
