Cervical and scapula-focused resistance exercise program versus trapezius massage in patients with chronic neck pain: A randomized controlled trial.

This was a four-week trial comparing two different approaches to chronic neck pain: a program of resistance exercises focused on the cervical spine and shoulder blade muscles, versus trapezius massage (the upper back muscle that often tightens with neck problems). Forty-one patients were split between the two groups and attended treatment five times a week. Both approaches led to measurable improvements in pain, neck movement, muscle tension, disability, and quality of life.
However, the resistance exercise group showed somewhat greater improvements across most measures, including pain reduction, certain directions of neck rotation, muscle tone changes, and overall disability levels. The authors propose that targeted strengthening may offer a meaningful edge over massage alone, though they acknowledge that longer studies are needed to understand how these effects hold up over time.

This RCT adds to the literature on mechanical approaches to CNP by directly comparing structured resistance training against a manual therapy baseline. The CSRE group showed statistically significant between-group advantages on pain, cervical rotation specifically, upper trapezius tone and stiffness, NDI, and SF-36 domains, suggesting a measurable but not transformative superiority. The study was limited to four weeks at high frequency (five sessions weekly), which is neither typical of many clinical settings nor informative about maintenance or deload phases.
The myotonometry data on tissue mechanical properties is a useful inclusion, though the clinical meaningfulness of those specific changes remains interpretable. Longer follow-up and exploration of dose optimization would strengthen the practical applicability of these findings.
