Efficacy of various exercise interventions for migraine treatment: A systematic review and network meta-analysis.

Researchers wanted to know which types of exercise work best for treating migraine headaches. They looked at 28 clinical trials involving 1501 people with migraines and compared how well different exercise approaches reduced how often migraines happened, how intense they felt, how long they lasted, and how much they interfered with daily life. Three forms of exercise stood out: yoga, high-intensity aerobic exercise, and moderate-intensity aerobic exercise all appeared somewhat more effective than medication alone at reducing how often migraines occurred.
When it came to pain intensity, only yoga showed a meaningful edge over medication. For shortening migraine duration, the two aerobic exercise approaches worked better than medication alone. The strength of the evidence overall was quite low, which means the findings are interesting but not yet definitive.

This network meta-analysis suggests that aerobic exercise and yoga occupy meaningfully different niches in migraine management. Yoga showed the most consistent advantage over pharmacological treatment across frequency and intensity measures, with effect sizes around 1.3 to 1.4 standardized mean difference units.
High and moderate-intensity aerobic work appeared more specific for duration reduction, with effect sizes ranging from 0.96 to 1.64. The confidence intervals are reasonably tight, suggesting these differences are unlikely to be noise, though the authors themselves rated the overall evidence quality as very low.
This gap between effect size consistency and evidence quality likely reflects heterogeneity in study design, measurement tools, and participant populations. The findings may be most useful for understanding which modalities warrant deeper investigation rather than as a basis for strong clinical recommendations.
