Adolescents With a High Burden of New-Onset Mood Symptoms After Sport-Related Concussion Benefit From Prescribed Aerobic Exercise, a Secondary Analysis of 2 Randomized Controlled Trials.

After a sport-related concussion, a notable share of teens develop new anxiety or depression symptoms that weren't there before injury, and this has been linked to slower recovery. This paper looks back at two earlier randomized trials comparing early prescribed aerobic exercise against a placebo-like relative rest program, asking whether the mood symptom burden a teen has right after injury changes how much exercise helps prevent prolonged concussion symptoms lasting a month or longer. The researchers split participants into those with a low burden of new mood symptoms and those with a high burden. For teens with high mood symptom burden, aerobic exercise noticeably lowered the chance of prolonged symptoms compared with the placebo group, a difference that was statistically significant.
For those with low mood symptom burden, exercise showed a smaller difference that was not statistically significant. This is an exploratory secondary analysis of existing trial data, not the studies' original main question, so the finding needs confirmation in research designed specifically to test it.

This is a secondary, exploratory analysis pooling two adolescent RCTs (ages 13-18, 2-10 days post-SRC) that originally compared individualized targeted heart rate aerobic exercise (n=102) against a placebo intervention mimicking relative rest (n=96), with incidence of persisting post-concussive symptoms (PPCS, ≥28 days) as the outcome. Here, the 198 analyzed participants were stratified by pre-randomization emotional symptom burden: 156 (79%) low burden (mean 1.2±1.65/24) and 42 (21%) high burden (mean 9.74±3.70/24). In the high-burden subgroup, exercise treatment was associated with a statistically significant reduction in PPCS hazard (HR 0.290, 95% CI 0.123-0.683, P=0.005, β=0.732).
In the low-burden subgroup, the hazard ratio trended similarly (0.767, 95% CI 0.546-1.079) but did not reach statistical significance (P=0.128, β=0.085). Because this is a post hoc subgroup analysis with a small high-burden cell (n=42), findings should be interpreted as hypothesis-generating rather than confirmatory, and the abstract does not address adverse events or safety monitoring during the exercise protocol. The distinction between the two subgroups' hazard estimates, rather than a single pooled effect, is the central and most clinically relevant detail for interpreting this trial series.
